Cataract Surgery with iStent

The iStent procedure is performed during the final stages of cataract surgery. This procedure is used to reduce intraocular pressure for patients with open-angle glaucoma.

Cataract surgery with iStent offers several advantages for patients with cataracts. iStent insertion is performed during the same procedure as cataract surgery, making it a minimally invasive option for patients. This means fewer incisions and less trauma to the eye, resulting in faster recovery times and reduced risk of complications compared to separate surgeries.

iStent is a tiny implant that is inserted during cataract surgery. It helps to reduce intraocular pressure (IOP) by improving the outflow of aqueous humor from the eye’s drainage system. By addressing both cataracts and glaucoma simultaneously, patients can potentially reduce their dependence on glaucoma medications and achieve better overall visual and ocular health outcomes.

Femtosecond Lasers (LenSx/Ziemer)

Femtosecond lasers, such as the LenSx and Ziemer systems, are advanced surgical tools used in various ophthalmic procedures, including cataract surgery and corneal surgery. These lasers utilize ultrafast pulses of light, measured in femtoseconds (one quadrillionth of a second), to precisely create incisions and perform tissue manipulation with exceptional accuracy and precision.

In cataract surgery, femtosecond lasers offer several advantages over traditional manual techniques. They can create precise incisions in the cornea, as well as capsulotomies (openings in the lens capsule) with micron-level precision. This level of accuracy can improve the alignment of intraocular lens implants and enhance visual outcomes following surgery. Additionally, femtosecond lasers can soften the cataract, making it easier to remove and potentially reducing the amount of ultrasound energy needed during the procedure, which can be beneficial for patients with certain risk factors, such as thin corneas or dense cataracts.

In corneal surgery, femtosecond lasers are used in procedures such as LASIK (laser-assisted in situ keratomileusis) and corneal transplantation. During LASIK, the laser is used to create a precise flap in the cornea, allowing for the reshaping of the underlying tissue to correct refractive errors such as nearsightedness, farsightedness, and astigmatism. In corneal transplantation, the laser can be used to create precise incisions in the donor cornea and recipient eye, facilitating better wound healing and visual outcomes.

Clear Lens Exchange (CLE)

CLE is typically performed on patients who are not suitable candidates for procedures like LASIK or PRK due to factors such as thin corneas, high refractive errors, or presbyopia. It can also be a preferred option for individuals over the age of 40 who are experiencing presbyopia and want to reduce their dependence on reading glasses or bifocals.

Pterygium Repair

Pterygium repair is a surgical procedure performed to remove a pterygium—a growth of tissue on the conjunctiva (the clear membrane covering the white part of the eye) that may extend onto the cornea. Pterygiums are often associated with exposure to ultraviolet (UV) light, dry and dusty environments, and other environmental factors.

SmartLipo

SmartLipo is a procedure that utilizes medical-grade lasers to create beams of light that are strong enough to rupture fat cells and then melt the fat.

Upper and Lower Blepharoplasty

Upper and lower blepharoplasty are surgical procedures aimed at rejuvenating the appearance of the eyelids by removing excess skin, fat, and muscle tissue.

Upper blepharoplasty, also known as upper eyelid surgery, focuses on addressing drooping or sagging skin in the upper eyelids. During the procedure, the surgeon makes incisions along the natural creases of the upper eyelids to access the underlying tissues. Excess skin, muscle, and sometimes fat are removed, and the remaining tissues are carefully repositioned and tightened to create a smoother, more youthful eyelid contour. Upper blepharoplasty can help improve the appearance of heavy or hooded eyelids, reduce the appearance of wrinkles and fine lines, and restore a more refreshed and alert appearance to the eyes.

Lower blepharoplasty, or lower eyelid surgery, targets puffiness, bags, and wrinkles in the lower eyelids. The surgeon may make incisions just below the lower lash line or on the inside of the lower eyelid (transconjunctival approach) to access the underlying tissues. Excess fat, muscle, and skin are removed or repositioned to reduce the appearance of under-eye bags and create a smoother, more youthful lower eyelid contour. Lower blepharoplasty can help improve the appearance of tired-looking eyes, reduce puffiness and dark circles, and restore a more youthful and refreshed appearance to the lower eyelids.

Both upper and lower blepharoplasty are typically performed on an outpatient basis under local anesthesia with sedation or general anesthesia, depending on the patient’s preferences and the extent of the procedure. Recovery time varies but generally involves some swelling, bruising, and mild discomfort, which can be managed with pain medication and cold compresses. Patients are typically advised to avoid strenuous activities and heavy lifting during the initial stages of recovery and to follow their surgeon’s post-operative instructions carefully to ensure optimal healing and results.

Nicholas P. Marsico, M.D., F.A.C.S. is a board certified ophthalmologist who performs all aspects of surgery on the anterior segment of the eye including cataract extraction, glaucoma surgery and various forms of corneal transplantation including penetrating keratoplasty and DSAEK.

Alfred Marrone, M.D. is a board-certified ophthalmologist and eye doctor specializing in reconstructive and cosmetic surgery of the eyelids and face.

Stephen, Dinsmore, M.D., F.A.C.S. is a board certified ophthalmologist specializing in cataract surgery with a focus on the use of the latest technology in multifocal intraocular lenses (IOLs).

Dr. Brar has been a board-certified ophthalmologist for more than 15 years, and has performed over 15,000 eye surgeries. He specializes in all-laser Custom LASIK, PRK, cataracts, intraocular lens implants and Wavefront technology.
